Students entering schools in Florida for the first time need a Florida Certification of Immunization form, also known as Form DH 680 or a “blue card.” This form must be completed by a physician or a Florida county health department. Form DH 680 may be printed on blue or white paper; as long as the form is certified, it is an official form.

For Marion County, all Form DH 680s must be picked up by a parent or guardian at our immunizations clinic at our Ocala Facility between 8 a.m. and 4 p.m. The Florida Department of Health in Marion County charges a $5 fee for each Form DH 680.
