DOH-Marion Community Health Needs Assessment (CHNA) Survey

Ocala, Fla. — The Florida Department of Health in Marion County (DOH-Marion), in conjunction with AdventHealth, encourages all Marion County residents to complete the 2025 Community Health Needs Assessment (CHNA) Survey. The survey is available in English, Spanish, Haitian Creole, Arabic, Russian, and Vietnamese. The survey is anonymous and open until October 31, 2024.

The survey consists of questions related to health care, mental health or substance misuse, food insecurity, and overall health and wellbeing. It takes approximately 15 minutes to complete. Results from the survey will help inform members of the Marion County Steering Committee for Health on how to improve the community’s health.
